• A list of participating Caremark National Network pharmacies
• A mail service order form and a pre-addressed envelope to obtain prescriptions by mail

About Your Prescription Drug Co-payment*
Please see your plan document and/or other plan correspondence for specific plan coverage and co-payment description.

Generic drugs typically have the lowest co-payment and are safe alternatives to brand name drugs. You may pay a higher co-payment for brand name drugs.

Caremark Mail Service Program
As part of your benefit plan, you have access to the Caremark Mail Service Program for maintenance type (long-term) prescriptions. The Caremark Mail Service Program provides benefits such as:

• A 90-day supply of drugs at one time
• Convenient delivery to your home, office, vacation spot or location of your choosing - including free standard shipping
• Toll-free number to speak to a licensed pharmacist  - 24 hours a day, seven days a week
• Time-saving online refills or refill-by-phone services - any time, any day

Getting started is easy…with two convenient options!

1. Call Caremark Customer Care toll-free at 1-866-772-9436 and ask for FastStart service. You will be transferred to a FastStart representative who will ask for the following information:

• Plan participant ID number
• Drug name
• Doctor name and phone number
• Shipping address
• Credit card and expiration date

2. That’s it! The representative will contact your doctor and fill out the order form for you.

OR

1. Get a 90-day prescription from your doctor, with as many as three refills (if appropriate).

2. Fill out the enclosed mail service order form and mail it in with your prescription and co-payment.

You can expect your prescription drug to arrive approximately 10 to 14 calendar days after Caremark receives your order.
